Apply for a place at Doutta Galla

Applying for a place at a Doutta Galla facility is a simple process and our people will be there to help you every step of the way.

Please see the steps below:

  1. Arrange an Aged Care Assessment
    Contact your doctor, social worker or aged care assessment service to arrange an aged care assessment.

  2. Arrange a Facility Tour
    Contact the facility to make an appointment and arrange a guided tour.  Contact details are listed on the individual facility pages in the Facilities section of the website.

  3. Complete the Application Form
    Following the guided tour, complete the application form that will be provided to you at the facility visit, together with the appropriate attachments and return it to the Facility manager.  A copy of the application form can also be obtained by downloading it from the Forms section of the website.

  4. Consultation
    We will then contact you to discuss financial details and availability of beds.  Following a meeting (by phone or face to face) we can determine if an offer of accommodation can be made.

  5. Offer of Services
    If an offer is made and accepted, a Resident Agreement will be provided for signing.

  6. Completed Agreement
    Return the signed Resident Agreement to Doutta Galla.  An admission date will then be discussed and organised.

  7. Welcome to Doutta Galla!
    Upon admission, an information session for the resident and family will be held and you will receive a Resident Information Guide.
